Section 3 - All teeth missing
If the patient is currently without all of their teeth, either wearing a removable denture or unable to function with a removable denture, several options are available.
Option One - The use of a Stud-attached Removable Over-denture.
This provides the simplest, most cost-effective solution and is firmly retained and stable but with similar dimensions to a well-manufactured removable denture. The patient needs to be comfortable psychologically with the continued use of a removable denture.

Option Two - Syncone Detachable Rigid Prosthesis
This system imported from Germany in recent years allows the provision of a rigidly attached denture (detachable bridge) to four implants placed into the lower jaw. The teeth will be securely fixed to the implants and will have little or no contact with the gum tissue, and the patient will be able to bite into normal foods.
In most situations if the current denture is of suitable manufacture and function, this denture can be converted into the Syncone Prosthesis. This has the benefit of providing an initially more cost-effective treatment with significant benefits in rigidity when compared to a removable over-denture (Option One). The patient must be aware that due to the significant increase in the chewing ability, the provision of a new prosthesis may be required together with a strengthener. However, the construction of this new prosthesis has similar procedures to the construction of a conventional denture or bridge. The costs of initial treatment including the conversion of the current denture are £5,000. The cost of a replacement lower prosthesis if required is £1,900. It should also be understood that the Syncone Prosthesis is detachable by the patient to simplify cleaning but should otherwise be worn at all times throughout the day.
Option Three - Fixed Implant Supported Bridge
This provides the nearest thing to natural teeth using implant technology. Implants can be installed into the remaining bone depending on the amount of available bone. The number of teeth that can be replaced will also depend on the amount of available bone. However, in most situations 10-12 teeth can be constructed. This will provide excellent function against either remaining upper dentition or the upper prosthesis, depending which is in position. Costs from £4,700

The costs of the bridge will depend on the number of implants placed and the type of bridge utilised. In many situations in the lower jaw it is possible to remove failing teeth, place implants, and provide the patient with fully implant-supported bridgework the same day as the teeth are extracted. This has significant advantages. Patients do not need to wear a removable temporary denture during the healing period. This technology is referred to as immediate loading.
