How to refer a patient
Simply complete the referral form and send it to us by fax, email or post. We will contact the patient to arrange an initial consultation. Following the initial consultation, a full written report will be sent to the patient, with a copy sent to you.
From then on we will keep you informed on progress. During treatment, patients will be reminded of the importance of continuing to see their own general dental practitioner for routine check-ups and any other necessary treatment. When the treatment has been completed, your patient will be returned to you for their continuing care.
